Add PHPMYADMIN_FORCE_SSL define to apache2 config, rewrite phpmyadmin URLs to https inside the the templates.

This is using the up-to-date list from Mozilla at
https://wiki.mozilla.org/Security/Server_Side_TLS

The comparison was mistakenly assumed to be a straight string
comparison, but in fact it is a regular expression.  And as we know any
string matches an empty regular expression.

D'oh.

This just makes the redirect a bit more reliable.

This removes a needless redirect when the HTTP_HOST is not the same as
the SERVER_NAME.

Fixes #12383

Also fixes the HSTS headers not getting sent due to wrong module name
in if clause (fixes #12343).
